Functions: · Coordination of services for the provision of home care for vulnerable adults and children · Reception and Training of new Community Workers / Auxiliaries · Ongoing follow-up of health personnel and monitoring of health care delivery · Patient assessment meetings and care plan definition · General administrative and coordination functions.
